Total Triage – Starting September

We’re introducing Total Triage from September to help improve access and make sure all patients get the right care at the right time.

What is Total Triage?

Total Triage means that all GP appointment requests will go through an initial assessment before being booked. This allows us to prioritise urgent issues and direct you to the most appropriate care – whether that’s a GP, nurse, pharmacist, or another service.

What does this mean for you at the moment?

  • If you need urgent help, we will assess and help you as quickly as possible.
  • If your request is non-urgent or routine, we will ask you to call back in September when the system goes live.

We understand this is a change, and we appreciate your support as we roll out this new system to improve our service.

We offer a number of different appointments to meet your needs. Sometimes it is important to see your GP face to face, sometimes all you need is some advice.

We offer telephone consultations, and video consultations where you can speak with your doctor on a video link via your smartphone, from work or from home.

Introducing total triage for all GP appointments

If you feel you need to be seen urgently on the day, our trained receptionists will conduct an initial assessment by asking you to describe the reason for your call. Where necessary, our receptionists might ask you to answer a few additional questions, designed by our GP team, to help our clinicians prioritise those patients who need more urgent care. Our GPs will review the information you have provided to decide which member of our healthcare team is best placed to help, the most suitable time frame for the appointment and whether this is best suited to be via telephone or in person at the practice.

Appointments are made for the Exmouth Health Centre, Treetops Surgery, Raleigh Surgery and Underhill Surgery by visiting reception at each site, by telephoning during office hours or by using our online services.

If you are unable to keep an appointment, please let our receptionists know as soon as possible.

Telephone Appointments

Quite often health related queries can be dealt with very quickly and effectively over the telephone. This allows for prompt assessment of your problem. Investigations can be booked, and we can discuss your results with you too.

Video Consultations

These are very similar to a face-to-face appointment. This is where you speak to a doctor using the video camera in your smartphone, tablet or computer.  This could save you a trip up to the health centre.

Nurse Practitioners and Practice Nurses

You may wish to consider booking an appointment with a practice nurse rather than a doctor. Practice nurses are qualified to deal with many aspects of your healthcare and you may be seen more quickly.

Sickness Certificates (Fit Notes)

You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home Visits

Whilst we encourage our patients to come into the surgery, where we have the proper equipment and facilities available, we understand that this is not always possible. Therefore, if you do need a home visit, you can help us by calling the practice before 10:30am.

You may only request a home visit if you are a housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to assess the urgency of your visit. If you are suffering with chest pains, breathing problems or severe abdominal pain, please make sure that you communicate it to us.

Chaperones

You are very welcome to be accompanied by a trusted friend or relative at your consultation if you wish, and this is all the more important if English is not your first language, or if you feel you might not be able to express what you need clearly to the doctor or nurse.

In additon, for intimate examinations, you will be offered the choice of having another professional clinician present at the examination. This is called chaperoning. This might not be possible at all times. If we cant allocate a chaperone when you wish one to be there, then we can reschedule your examination to another time.

Out of Hours

Extended Hours

Life is busy and your time is precious. We are happy to offer you appointments outside of normal working hours. We offer appointments in the evenings and on some Saturdays. If you would like to book one of these appointments, please phone reception on 01395 226540

Life Threatening

Call 999 or go to A&E now if:

  • you or someone you know needs immediate help
  • you have seriously harmed yourself – for example, by taking a drug overdose

A mental health emergency should be taken as seriously as a medical emergency.

111 is the NHS non-emergency number. It's fast, easy and free. Call 111 and speak to a highly trained adviser, supported by healthcare professionals. They will ask you a series of questions to assess your symptoms and immediately direct you to the best medical care for you.

NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones.

In case of a life-threatening emergency, please dial 999.

There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
