Abtrace: A new way to book your health monitoring appointments

What is it?

We will be changing the way we invite patients for their health monitoring appointments, for example diabetes reviews, medication blood tests or vaccinations. We will be using a piece of software for proactive monitoring called ‘Abtrace’.

If you currently receive texts from our team, they will start to come from ‘GPSurgery’. Booking links will look a little different too. Please click on the link sent within your text, which will allow you to log in to your Patient Portal Page.

Here you will be able to book your monitoring appointments and view pending health tasks. If required, you will also be able to send us your own reading for blood pressure or tell us other information such as your weight and current smoking status.

I don’t have a smartphone / internet access? 

If we cannot contact you by text message or email, you will receive a letter in the post as usual
