Community Midwives

The community midwives, The Palm Team, are based at Exmouth Hospital and can be contacted on 01395 282114.

Community Nurses

Public Health Nursing Team

The public health nursing team (Health Visitors/School nurse) are based at Devon County Council, and they can be contacted on 0333 234 1903.

District Nurse Team

The District Nursing Team continues to provide high quality research-based care for patients who are house bound and are unable to access surgery facilities. We are integral part of the wider healthcare team, social services and other agencies. We support complex packages promoting early discharge from hospital and preventing hospital admissions.

We are a pro-active team promoting innovative district nursing services, maintenance of health and wellbeing, management of chronic diseases, holistic assessment of nursing needs, care of the dying and supporting the vulnerable older person at home.

The District Nurses can be contacted on 01395 282079, please note this phone is often unmanned when the nurses are out visiting. Please leave a message and the team will get back to you routinely. If your need is more urgent please contact the Practice.
